Policies, Disclosure of Information, and Informed Consent:  

Dr. Patricia Pimentel Selassie graduated from a five-year accredited Naturopathic Medical school.  Currently, there are 26 jurisdictions, including the District of Columbia, Puerto Rico and the US Virgin Islands which have laws regulating Naturopathic Doctors to practice medicine, including diagnosing, treating, or prescribing for patients. Legislation is pending in New York State.

Dr. Selassie is independent of any hospital affiliation. Therefore, I understand that I am advised to maintain contact with a local physician or facility for acute and emergency care, diagnostic procedures, laboratory evaluations and monitoring of chronic illnesses or medications.

I understand that Dr. Selassie and her associates will make the best possible effort to comply with all HIPAA regulations.  I also understand that, despite their best efforts, it is possible that confidential information might be inadvertently disclosed to unauthorized parties.  In the event of such an occurrence, I agree to hold Dr. Patricia Pimentel Selassie and her associates harmless.

Naturopathic medicine differs from allopathic medicine by the large blocks of time that a patient receives.  Patients are asked to make every effort to keep their appointment, or else provide 24-hours’ notice to cancel.  As a courtesy, a patient in a Program/Package, the first missed appointment without 24-hour notice will NOT incur a fee. However, after the first missed appointment, I understand that failure to provide 24-hour notice for a missed appointment going forward WILL incur a no-show fee equal to the cost of the appointment.  

I understand that if I want to take advantage of Dr. Selassie’s packages or programs, payment in full or the first installment is expected at the time of commitment. Please keep in mind that these Package/Program fees are non-refundable.

I understand that all Package/Programs should be completed within the allotted time. However, a grace period of an additional one year is given to complete the Package/Program.  After the grace period is completed, the package is officially expired.

Please note that Dr. Selassie does make a profit from selling supplements through her store.  It Is NOT a requirement that you purchase your supplements through the store.
